Not really ghoulish... more just... not tasteful. Can't think of a better word. I long for the days of pregnancy muumuus. Don't know why extra attention is brought to the belly these days.

For the type of people who would be wearing this (clearly: PREGNANT ones), I don't see why it should be bother people. Little wee ones inside do have skeletal structures. Even pro-choicers should be aware of that.
The shirt doesn't make a political statement. It's funny.
